Procedures to Obtain Medical Records

To obtain medical records, Madison Parish Hospital requires that you:

• Have Personal Photo Identification with you
• Complete, sign and date an Authorization for Release of Protected Health Information form

Prices

• The price of medical records obtained for Legal, Insurance, Personal, or other reasons is $6.50 + ($0.05 per page). 
• If the medical records are obtained for medical purposes (ex. To be faxed to another facility, doctors, continuing treatment, etc.) there is no charge. 

Our medical records building is a white building located at 1000 Johnson Street, Tallulah, LA. This building shares a parking lot with our facility.
