Case Management

Case Management is a collaborative process that assesses, plans, implements, coordinates and evaluates the options and services required to meet a patient’s individual needs.  Here at Madison Parish Hospital, the Case Manager works closely with the Social Worker to provide any assistance needed after discharge, i.e., Home Health, Hospice, Skilled or LTC facility placement, etc.
